Checklist for Completing Cotillion/Masters’ Ball Application
Your application package must be completed and submitted with the application fee by the deadline (December 3) before you can officially participate as a Prospective Debutante or Master.
- Complete the Debutante/Masters Application in full with all necessary signatures.
- Fill in the top of the three Recommendation Forms and give to (3) different individuals for personal references.
- The respondent can send the recommendation form directly to the Foundation’s P.O. Box address no later than January 7.
- Enclose a copy of your most recent report card (first quarter).
- Return the Debutante/Masters’ application, the application fee and the copy of report card to the December 3rd meeting or to the address below by the JANUARY 7.
